Non conosco un modo per farlo in FOP, ma ciò potrebbe essere dovuto al fatto che non ho usato FOP per qualcosa di più della semplice generazione di PDF.
Utilizzando RenderX (XEP), è possibile impostare fo:table-header su qualsiasi titolo che si desidera utilizzare sulle pagine continue e utilizzare un normale fo:block per il titolo iniziale.
È quindi possibile utilizzare l'attributo rx:table-omit-initial-header="true" su fo:table per non emettere l'intestazione della tabella quando la tabella viene emessa per la prima volta.
